Your Union Health Benefits are managed by the DC 37 Health & Security Plan. To get the most out of your benefits, ensure you are enrolled and that your information is up-to-date.
Information on coverage and how to apply for all Health & Security Plan benefits may be obtained by calling the Inquiry Unit at (212) 815-1234. Be sure to have your member PID number or Social Security number handy when you call.

Beneficiary Designations: Update your Beneficiary Designations Form (PDF, 292.52 KB) to ensure that the beneficiaries on file are accurate and current.
